It's simple. Fill out the form below so we may contact your friend to see if he or she may qualify for Disability Benefits. If your friend meets SSDC's minimum eligibility requirements and uses SSDC to file their claim to win his or her benefits, we'll send you a check for $150.
By submitting this information you are acknowledging that SSDC will contact your friend or relative. Please be respectful to your friend and check with them first if it's ok to provide SSDC with their contact information.
